Fascilities Request Form April 9, 2008

To get something listed on the Incarnation Parish calendar, a Facilities Request Form needs to be submitted (for regular, ongoing events, a separate form is not needed for each of them). I have attached a copy of the Form that must be filled out.  Feel free to type the information into the form (handwritten forms are not required).  Then, email the form to lneill@incarnation.org and copy bsteele@incarnation.org and molver@incarnation.org on that email.
